Stanford Patient Education Research Center is recruiting participants for the Active Living with Chronic Conditions Tool Kit Program and Research Study.
Participants will receive the book Living a Healthy Life with Chronic Conditions, an exercise CD, a relaxation CD, and self-management tip sheets. This program is designed for people who cannot, or choose not to, participate in a community based Chronic Disease Self-Management Program (CDSMP) workshop and was developed to be a “mail delivered corollary” to CDSMP and the original small group in this study experienced improved health status and reduced health care utilization compared to the control group.
