Augusta Health accepted applications for single year grants in the fall this year. They requested that the grants address one of the priority areas defined by their recent Community Health Needs Assessment. One of those priorities is diabetes.
So the VPAS Chronic Disease Self Management Education (CDSME) Program Coordinator Joyce Nussbaum wrote a grant request with some assistance from SAW Regional Director Janice Gentry and CEO Paul Lavigne focused on expansion of the Diabetes Self Management Program (DSMP) in the SAW Region.
On Dec. 20, we received word that the grant request was fully funded at $10,000 and that we should receive the check next week. Janice and Joyce will meet after Christmas to start planning for implementation of this program.
