VPAS Begins Search for New Case Manager in Highland

Highland Community Case Manager

Days: Monday – Friday, varied

Hours per Week: average of 6 hours/week

Closing Date: August 9

Start: Immediately

 

RESPONSIBILITIES:

The Case Manager: supports Highland community residents 60 years and older to remain in their own home; is the gatekeeper to VPAS services such as Meals on Wheels, Personal Care, and Caregiver Support; conducts home visits and a comprehensive assessment using the VA Uniform Assessment Instrument (UAI) to determine program eligibility and identify unmet needs; identifies, coordinates, and arranges services to meet the person’s needs; monitors the effectiveness and utilization of services by conducting annual reassessments and other contact as needed; works with the Highland County Local Council to ensure comprehensive services to Highland community residents; and assists Medicare beneficiaries with questions, appeals, and enrollment issues as a Certified VA Insurance Counseling and Assistance Program Counselor.

QUALIFICATIONS:
Bachelor’s degree in Social Work or human services related field or equivalent experience; at least two years of experience working with older adults; knowledge of local community resources; strong detail-oriented skills in assessment, interviewing, record keeping, documentation, data entry; excellent verbal and written communication skills; strong organizational and problem solving skills; ability to work both independently; ability to learn complex material and explain it in understandable language; desire to update skills as aging services evolves; working knowledge of Word, Outlook and ability to learn a web-based application.
