Health Risk Assessment Grant Awarded

The Rockingham Memorial Hospital (RMH) Foundation has awarded a grant to the Virginia Mennonite Retirement Community (VMRC), VPAS, and Sentara RMH Community Health. The goal is to “provide a Health Risk Assessment to adults who are physically, socially and/or culturally isolated in order to identify risk for diabetes, cardiovascular disease (including stroke) and hypertension and provide education, referral and follow up.”

This Health Risk Assessment will be offered to 100 persons including VMRC residents, VPAS Senior Center Members, staff and community members. Measurements will include A1c to measure blood glucose, height and weight to measure Body Mass Index (BMI), blood pressure, tobacco and alcohol use history, family history, level of physical activity and nutrition. Based on results , which will be provided to participants immediately following the assessment, referrals will be made to smoking cessation programs, weight management programs and Chronic Disease Self Management Education (CDSME).

This is also an RMH and JMU collaborative effort involving IRB approval, data collection and evaluation that will provide additional data for future grant applications.
