CDSME Workshop Registration Available Online

cdsmp-logoRegistration for Chronic Disease Self-Management Education (CDSME) workshops can now be completed by visiting the CDSME webpage online at http://valleycdsme.com and filling out the information under the Workshop Schedule/Register tab.

The information entered will be sent immediately to the CDSME Coordinator and a confirmation will be returned by email.

This option is for anyone who would like to attend a workshop and can also be used by others such as Workshop Leaders and VPAS Staff who would like to register potential participants who have contacted them.
