Chronic Disease Self-Management Education (CDSME) Coordinator Joyce Nussbaum attended the Woodrow Wilson Rehabilitation Center (WWRC) Life Skills Transition Program Completion Ceremony on Friday, April 25.
The Life Skills Transition Program (LSTP) is a 9 week program that provides people with disabilities an opportunity to learn more about skills to prepare them for employment and independence. WWRC began including Chronic Disease Self-Management Program Workshops in the LSTP curriculum in July 2013 and has had more than 150 students complete the program since then. Rick Sizemore, WWRC Director, expressed appreciation for the opportunity to partner with VPAS CDSME.
It was encouraging to hear the LSTP graduates share what they have learned and what they will be doing next and it is exciting that CDSME can be a small part of this wonderful program at WWRC.
