Fourteen members of the Bath Community Senior Center completed the Chronic Disease Self-Management Program – Diabetes Self-Management on July 28 and received a Certificate of Completion.
Our members learned valuable information about Diabetes and how to manage their chronic condition more effectively and improve their health.
We would like to thank the facilitators of this group, Perlista Henry, BCSC member and VPAS Board member and Mary Albert, Nutritionist from Bath Community Hospital, as well as Joyce Nussbaum, VPAS Chronic Disease Self Management Education Coordinator for scheduling this workshop.
