CDSME Presented to POMA at Augusta Health

augusta-health-headerChronic Disease Self-Management Education (CDSME) Coordinator Joyce Nussbaum presented information about CDSME to 21 people at the Physician Office Managers Association (POMA) meeting at Augusta Health June 18.

Joyce focused on the immediate need for Leaders for the Diabetes Self-Management Program (CDSMP) Leader Training, informed office managers about the benefits of the program for both the practice and the patients, and asked for their assistance throug locating sites and patient referrals.

Dana Breeding, RN Health Educator Community Wellness at Augusta Health, joined Joyce for the presentation to clarify that this is a partnership with Augusta Health and that she and Joyce are working together to provide workshops at Augusta Health and throughout the community.

Dana also shared that she and Joyce just finished co-facilitating a DSMP Workshop at Augusta Health.
