Volunteers Sought For Disease Management Program

vpasheader0311Leader Training for the “You Can! Live Well, Virginia!” Chronic Disease Self-Management Program (CDSMP) will be held on January 17, 18, 24 and 25 from 9:00 – 4:00 at the Carilion Stonewall Jackson Hospital in Lexington. Light snacks, lunch and all necessary materials will be provided.

This free four-day training provides participants with the skills and certification necessary to be volunteer leaders in the “You Can! Live Well, Virginia!” Chronic Disease Self-Management Program (CDSMP), which is an evidence-based program developed and licensed by Stanford University. CDSMP workshops are co-led by two trained volunteer leaders and are held for 2 ½ hours one time each week for six weeks. Workshops are designed to help older adults and adults with disabilities with one or more chronic conditions learn ways to actively manage their condition, take charge of their life, and improve their health status.

Workshop Leaders should have a chronic condition or be a caregiver for someone with a chronic condition and be willing to facilitate the course as outlined in the Stanford Leader Manual and lead at least one workshop within 6 -12 months of training.
