CDSMP Leader Training Scheduled in Lexington Beginning Jan. 17

newspaper-headerThere will be a Leader Training for the Chronic Disease Self-Management Program (CDSMP) January 17, 18, 24 and 25 from 9:00 – 4:00 at the Carilion Stonewall Jackson Hospital in Lexington.

Light snacks, lunch and all necessary materials will be provided. This 4 day training provides participants with the skills and certification necessary to be leaders in the “You Can! Live Well, Virginia!” Chronic Disease Self-Management Program (CDSMP) which is an evidence based program developed and licensed by Stanford University (http://patienteducation.stanford.edu/programs/cdsmp.html).

CDSMP workshops are co-led by 2 trained leaders and are held for 2 ½ hours one time each week for six weeks. Workshops are designed to help older adults and adults with disabilities with one or more chronic conditions learn ways to actively manage their condition, take charge of their life, and improve their health status.

Workshop Leaders are volunteers and must be comfortable speaking in front of a group, have a chronic condition or be a caregiver for someone with a chronic condition, be willing to facilitate the course as outlined in the Stanford Leader Manual, and be willing to lead at least one workshop within 6 -12 months of training.
