Bath Senior Center members completed CDSME-diabetes program

bathlogolargeFourteen members of the Bath Senior Center completed the Chronic Disease Self-Management Program (CDSME) and the Diabetes Self-Management Program (DSMP) on July 28 and received a Certificate of Completion. Participants learned valuable information about Diabetes and how to manage their chronic conditions more effectively and improve their health.

Many thanks to the facilitators of this group, Perlista Henry, BCSC member and VPAS Board member, and Mary Albert, Nutritionist from Bath Community Hospital, as well as Joyce Nussbaum, VPAS Chronic Disease Self Management Education Coordinator for scheduling this workshop.
