Augusta Health single-year, multi-year grant applications available

augusta-health-headerAs part of its mission to promote the health and well-being of its community through access to excellent care, Augusta Health provides single-year and multi-year grants to local non-profit and governmental community organizations.  Since 2002, Augusta Health has awarded more than $1 million in gifts and grants to local schools and organizations.

This year, applications will be available on, and submitted through, Common Grant Application.  The link for accessing the grant application is on the Augusta Health website at www.augustahealth.com/community-outreach/single-year-grants or www.augustahealth.com/community-outreach/multi-year-grants. The applications must be submitted online by midnight, July 31, 2015.

To help applicants with understanding the grant application process and online application, a Grant Process Orientation Meeting will be held on Thursday, June 4, 2015 from 9-11 am at the Augusta Community Care Building.  Please contact Rhonda Clifton (540.332.4190 or [email protected]) to make a reservation for this meeting.  A You Tube video has also been created for Augusta Health grant seekers.

Grants are awarded for amounts greater than $2500.  Requests of funding for projects less than $2500 are considered monthly through the Community Contributions process.  That form is available at www.augustahealth.com/community-outreach/community-contributions.

Projects and initiatives receiving Augusta Health grants must address one of the community health priorities identified in the most recent Community Health Needs Assessment (CHNA).

The grants provided by Augusta Health are overseen by its Community Benefit Committee and are part of a structured plan to meet the ultimate purpose of any CHNA—to improve our community’s health.

The latest CHNA is available on Augusta Health’s website at www.augustahealth.com/community-outreach/community-benefit-program.  The priorities identified in this report are:

  • Chronic Disease Management—Obesity (including underlying factors of nutrition and physical inactivity); Diabetes; Cancer; Heart Disease
  • Health Behaviors and Community Education—Taking personal responsibility for obesity, nutrition and physical inactivity; increased health education and programming related to diabetes, obesity (nutrition/physical inactivity) and wellness
  • Access and Affordability–Access to care, especially for low income residents and the uninsured, affordable health insurance, overuse of the emergency department
  • Behavioral Health—Integration of mental health services and primary care; depression; teen suicide; substance abuse; access to mental health care and substance abuse treatment services
  • Socioeconomic Factors (social determinants of health)—Low income/poverty; Low educational attainment

Those awarded grants will receive, in compliance with IRS regulations, a letter restricting the funds provided to the community benefit purpose submitted in the grant.  Reporting will be required.
