Augusta Health grant applications available

augusta-health-headerAs part of its mission to promote the health and well-being of its community through access to excellent care, Augusta Health provides single-year and multi-year grants to local non-profit and governmental community organizations.  Since 2002, Augusta Health has awarded more than $1 million in gifts and grants to local schools and organizations.

Both Single-Year and Multi-Year grant applications are now available on the Augusta Health website.  The applications are due to Augusta Health by July 15, 2014.  Grant awards will be announced on September 2, 2014.

Projects and initiatives receiving Augusta Health grants must address one of the community health priorities identified in the most recent Community Health Needs Assessment (CHNA). The community health priorities identified are:

Chronic Disease Management:  Obesity, Diabetes, Cancer, Heart Disease, and Nutrition/Physical Activity

Health Behaviors and Community Health Education:  Health education and programming, particularly for diabetes, obesity and wellness

Access and Affordability:  Access to primary care providers, particularly for Medicaid and Medicare beneficiaries, low income residents and the uninsured

Behavioral Health:  Depression, Teen Suicide, Substance Abuse, Access to mental health and substance abuse treatment services

Socio-economic Factors:  Low income, Low educational attainment

Augusta Health provides single-year grants to non-profit and governmental community partners.  The grant process and guidelines for Single-Year Grants are at www.augustahealth.com/community-outreach/single-year-grants.  Last year’s recipients were the Augusta Regional Free Clinic; The Boys & Girls Club of Waynesboro, Staunton and Augusta County; Crossroads to Brain Injury Recovery; Highland Medical Center; Project GROWS; Rockbridge Area Transportation System; Valley Hope Counseling Center; Valley Mission and Valley Program for Aging Services.

Augusta Health also provides multi-year grants, three years in length, to non-profit and governmental community partners who develop a project that requires more sustained funding to implement.  Organizations that apply for multi-year grants must demonstrate a history of successful programming, collaboration with other agencies and the ability to monitor outcomes and impact.  The grant process and guidelines for Multi-Year Grants are at www.augustahealth.com/community-outreach/multi-year-grants.

Both Single-Year Grant and Multi-Year Grant application requirements include identification of measurable outcomes, a plan to evaluate outcomes and submission of progress reports.
